Description
We are looking for Community Social Workers to join our amazing community team
Responsibilities:

Support Social Work team in providing high level of psychosocial assessments and discharge planning, ongoing assessment and individualised support to patients and their family members based on identified needs in a way that empowers patients to find appropriate solutions, promotes independence and improves their health
• Conducts patient psychosocial assessments to determine needs/goals and makes treatment recommendations for a variety of patients including transplant, oncology, emergency department, etc.
• Provides consultation and collaborates with physicians, nurses and other health care team members to assist in the development of a comprehensive and coordinated plan of care
• Engages patients and their families in adhering to the treatment plan by reducing or eliminating, to the extent possible, social, psychological, financial and behavioral barriers to successful outcomes
• Assists Case Manager and Assistant Social Worker with complex social situations
• Advocates on the patient’s behalf for access to appropriate post- acute care services
• Providing crisis intervention as needed Identifies and assists the under or uninsured patient to locate available resources and advocates with payers and social agencies to locate sources of financial support
• Acts as a resource to CCAD personnel on a wide range of social and cultural topics Assist in the provision of emotional support and education for both patients/families and caregivers when required
Requirements
Experience working with clients that are affected by issues including neglect, child abuse, domestic violence, mental health, and parental substance abuse.
Ability to give full attention to what social service clients are saying, understand the points being made, ask questions, and not interrupt inappropriately.
Awareness of others’ reactions and understanding why they react as they do.
Ability to judge the relative costs and benefits of potential social service and health care actions and choose the most appropriate one.
Comfortable using a computer for various tasks.
Proficient in word, excel, outlook, and powerPoint.
Ability to stay calm and empathize with clients when they are upset.
Able to develop a treatment plan.
Knowledge of crisis intervention.
