- We are looking to hire a Health Coordinator to support the planning, implementation, and management of health programs aimed at improving community and patient outcomes. The ideal candidate will be organized, proactive, and skilled in coordinating healthcare services and resources.
Key Responsibilities:
Plan, develop, and oversee health programs and initiatives in collaboration with healthcare teams.
Coordinate patient care services, ensuring smooth communication between patients, providers, and support staff.
Track and analyze health data to evaluate program effectiveness and identify areas for improvement.
Organize health education and outreach activities to promote wellness and disease prevention.
Maintain accurate records and reports related to health programs and patient follow-ups.
Liaise with community organizations, government agencies, and healthcare providers to optimize resource use.
Assist in scheduling appointments, referrals, and follow-up care for patients.
Support compliance with healthcare regulations and quality standards.
Facilitate training and development sessions for staff and volunteers as needed.
